Instructions for Warranty Claim


1.) Fill out the form to the right. Once completed we will send you a pre-paid, pre-filled UPS return label via email within 1 business day. Please print and use this to ship back the package.
2.) Your return authorization code is the order number you received when you initially bought the product.  Please make sure the outside of the package is marked with this code.  The easiest way to locate this number is to search your email or login to your account.  If you are still having trouble, please call 1-866-445-4128 option 2 and we will help locate your order number.
3.) Once we receive your package, we will ship out your replacement product within 48-72 hours.
